Can't progress past Erland's License Manager

Other LMS users have suggested I try Erland's Dynamic Playlist plugin to overcome some of the LMS playlist limitations. I downloaded and installed that playlist, along with the License Manager that is required to activate it, but can't get past the Settings screen of License Manager.

The two screen-dumps show the problem, and I *do* have a MySqueezebox.com account.

Any help would be appreciated, as I'd like to trial this plugin for a month before deciding whether to buy a license.
