Webhook / HTTP Plugin - Does One Exist?

We're currently developing Squeezebox control for the Athom Homey home automation device and as part of this, we'd love to add the ability for Homey to react when the track changes, the device is paused from the controller, or the state otherwise changes externally.

The best way (and I'm open to other ideas) seems to be if there is a plugin which will send out an HTTP request on certain events (e.g. track change) that we could point at the Homey API endpoint for our app. Does anyone know if an HTTP / Webhook plugin already exists? It seems like we shouldn't be the first to think / request it but I'm having trouble finding anything.

It might be that there is a better way to achieve this (polling doesn't seem like a good way!) so I'm open and grateful for any and all input. Maybe there is even a way for our Homey App to register itself for HTTP updates or something along those lines?
